Initial Assessment and Treatment

This appointment includes Intake, Assessment, and Treatment that takes approximately an hour. Please schedule this appointment if this is you're first time seeing this practitioner. Please fill out the Health History Form and Agreements for treatment. A friendly reminder that we require 24 hours notice for rescheduling or cancellations. A 50% cancellation fee will apply in the event of a cancellation within 24 hours of the scheduled appointment time, or in the event of a no-show. Please note: Only accepting cash or E-Transfers at this time.
